What is MyChart?

MyChart offers patients personalized and secure on-line access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With MyChart, you can use the Internet to:

  • View your test results and health summary from the MyChart electronic health record.
  • Access trusted health information resources.
  • Request Prescription Refills
  • Request appointments at your physician's office
  • Communicate with your providers online

How do I sign up?

Patients who wish to participate can sign up in multiple ways. To sign up online, complete the sign-up form as accurately and as completely as possible and click Submit. Click here to access the form to sign up online. If the system is unable to accurately match you to your patient record you will be issued a MyChart activation code. This code may also be requested from your nurse during your hospital visit, from the Health Information Management department at the hospital, or from your Community Care Network doctor’s office. This code will enable you to login and create your own user ID and password.

Who do I contact if I have further questions?

For questions or support in the use of MyChart, please contact our MyChart Customer Service Line at 219-226-2313. They will request your name and a phone number where you can be reached. Someone from the MyChart team will contact you regarding your questions.

Does my provider use MyChart?

If your provider is affiliated with Community Care Network they are utilizing MyChart. To verify affiliation, click here and search for your provider's name. In the View Profile section you'll see the MyChart status under Additional Information.

If some of my health information on MyChart is not correct, what should I do?

Your MyChart information comes directly from your electronic medical record. Should you find any discrepancies, please contact our MyChart Customer Service Line at 219-226-2313.

Why don't I see any health maintenance reminders in MyChart?

MyChart health maintenance is only displayed for patients that are seeing a primary care physician that is part of the Community Care Network. Click here to search for your provider and look for the Community Care Network identifier. If you believe this is an error, please contact your provider's office to have your primary care physician updated.

Does my provider get my lab results too?

Your provider receives your lab results when they are finalized. If they are a Community Care Network provider they will receive them electronically. External providers will receive them either electronically or via fax.

How may I view a family member's health record in MyChart?

MyChart - Proxy Access allows a parent or guardian to log into their personal MyChart account and then connect to information regarding their family member.

What is MyChart - Proxy Access?

MyChart - Proxy Access gives parents or legal guardians of minor children access to portions of their child's medical records

MyChart - Proxy Access can also be used to access portions of your spouse's or dependent adults medical record.

For your convenience, you can access and complete online proxy request forms by logging in to your MyChart and clicking the "Request Family Access" link under the Messaging tab. Available online proxy form requests include: Access to a minor's MyChart, Granting access to your MyChart, and Restricting access to a minor's MyChart.

Optionally, you may complete the MyChart Child Proxy Form or MyChart Adult Proxy Form and return it to your provider's office to request access to this convenient service.

Can I ask questions regarding a family member from my MyChart account?

MyChart offers direct access to your personal health record and communicating about another individual's information would be placed in your health record. This information would not appear in the correct health record and could potentially jeopardize medical care.

Can my spouse and I share one MyChart account?

No, due to the sensitive nature of medical information, each adult must sign and submit a Release of Information request and establish their own MyChart account.

I forgot my password. What should I do?

You should click the "Forgot password" link on the sign-in page to reset your password online.

Can you send me a new activation code as I have lost it or did not receive it?

You can request a new activation code or sign up onine by clicking here and filling out the required information. If we are unable to accurately match you to your patient record a new activation code will then be emailed to you. You may also request a new activation code at your physician’s office.

Where can I update my personal information (e.g., e-mail, demographics, or change my password)?

To change your password, log into MyChart and select the Settings tab, go to the Security settings section and update your password. Make sure you click Save Password prior to leaving this page. To update your demographics, log into MyChart and choose Demographics from the Health tab. Here you can update your address, phone number, email address, and some other demographic fields.

How is MyChart secure?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal ID's, and passwords. Each person controls their password, and the account cannot be accessed without that password. Further, MyChart uses the latest 128-bit SSL encryption technology with no caching to automatically encrypt your session with MyChart. Unlike conventional e-mail, all MyChart messaging is done while you are securely logged on to our website.

What is your Privacy Policy?

MyChart is owned and operated by Community Healthcare System and is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold or leased.

I was logged out of MyChart, what happened?

We aim to protect your privacy and the security of your information. While logged into MyChart, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of MyChart. We recommend that you log out of your MyChart session if you need to leave your computer for even a short period of time.

What do I need to use MyChart?

You need access to a computer connected to the Internet and an up-to-date browser (such as Internet Explorer 11, Safari 6+, Firefox 33+, Chrome 38+).

Is my activation code my user ID?

No, your activation code is not your MyChart ID or password. You will use this code only once to log into MyChart for the first time. When you log into MyChart the first time, you will then be asked to create your own unique MyChart ID and password.
